Hello everyone, I'm new here so this is my first post... I started having very mild ED issues 3 years ago (but i still had great libido at the time). At that time I would be having sex and after about 5-10 minutes ED would happen. I started taking a multivitamin & as long as I took the multivitamin ED was never an issue. When ED would happen after apologizing to my girlfriend I would realize I had forgotten to take my multivitamin for the past few days. A few days after taking them every morning & i was back to normal-no ED.

Fast forward two years later (so about march of 2016). I'm still taking the same multivitamin ( [edited by mod] - Vit A, C, D, E, niacin, B6, Folic acid, B12, Biotin, chromium, molybdenum, Na, pantothenic acid) and not having ED if i took it. Then over the course of lets say 5 days or less my libido went from needing a sexual release 3 times a day to NONE. When i say none, i mean zip, zero, nada. Dream girl, sexual fantasy nothing in the world could turn me on. My penis didn't feel "numb" but it felt different.. Again it wasn't a numb feeling, but it was like it had no feeling & was typically shriveled up (so no blood in it).. I would have to try really hard to get an erection & if i stopped touching myself it would go away within 30 seconds. 

I went to see the Urologist and an endocrinologist. They ran TSH, free & total T3, Free and total t4, testosterone, and prolactin. Everything was normal both times. And offered no suggestions or place to turn to next.

I'm a fit guy (5'10'' 155 lbs) but wasn't the best eater (basically just meat & bread) & not that active. About a month ago I started running along with eating fruit at lunch or dinner every day. I also started taking 2 grams of L-Arginine and 2 grams of Critulline. This seemed to help some (in the ED department not really my libido). I started getting morning erections again (but i wasn't "horny" when i got them in the morning like i used to be, I just got them). This has certainly helped me when I force myself to get an erection, but it won't last one minute inside of my girlfriend. I feel like if i was horny ED wouldn't be a problem, I just don't have any libido. I'm not sure if that's my problem or if no libido is a result of ED.

I've been trough the website & read other articles I've considered trying Rhodiola Rosea, CoQ10, Ginseng, Maca, Tribulus and DHEA. And then suppllements like [edited by moderator] which are basically a mixture of the above supplements (with other ingredients). I just don't really know which one to try...

So my question for you guys is there something else I should check in the lab? What supplements should I try? When i try a supplement how long should you take them before you decide it didn't help?

Posting your labs and the lab ranges/values would generate a more focused conversation. Dr's often will look and tell you that a TSH of 4.5 is fine and it's far from being fine, so when a guy says things are normal...

Glucose-97 mg/dL     
Prolactin-9.3 ng/mL                 Range 2.1-17.7
TSH-1.79 ulU/ml                      Range 0.35-5.50
Free T4- 1.18ng/dl                  Range 0.72-1.55
Free T3-3.29 pg/mL                Range 2.3-4.2
Ferritin -138.5 ng/mL              Range 22.0-322.0
Testosterone has been checked on 3 different occasions over the past 3 years and is always about 400

If that's 400 ng/dL then somebody should have mentioned that it's pretty low for your age. In the 600s is normal. 400 is average if you're in your 80s maybe.

In any case, it would be good to know your SHBG. This would be used to calculate free testosterone, the most usable kind and a better reflection of your condition than total.

I would love to find a root cause. I should mention I was diagnosed with Leukemia at age 9 and went through 1.5 years of treatment (chemo) I'm not sure if that plays into this, but i know the drugs i was treated with can cause thyroid problems later in life.

400 ng/dL is low, take it from me who has the same issues as you and am 350 ng/dL in the morning and I am just 2 years older than you.  Doctors will tell you that anything within that super wide reference range is normal when in reality, it is low.

See if you can get your LH and FSH tested as well.  This will help pinpoint what kind of pattern you have.

BTW, I don't mean to tell you how to live your life, but some would consider that 3 sexual releases daily would be pretty excessive and exhausting to the body.  From what I have read, adrenal exhaustion is a thing and it can contribute to reduced sexual desire.

SHBG --> 22/19 (10-57)
LH --> 4.3/3.1 (1.7-8.6)
FSH --> 1.7/1.6 (1.5-12.4)
Prolactin --> 5.1 (4-15.2)
Estradiol --> 11.42/24.06 (7.02-49.06)

Started taking clomid September 2016.  Tested Oct 5 and November 30:

Total T --> 649/502 (249-836)
Free T --> 149/123 (30-150)
SHBG --> 26/21 (10-57)
LH --> 7.7/7.3 (1.7-8.6)
FSH --> 2.3/1.9 (1.5-12.4)
Prolactin --> 11.3/9.4 (4-15.2)
Estradiol --> 29.49/26.32 (7.02-49.06)

Off clomid Jan 16, 2017 due to no improvement in symptoms.

Current symptoms:  low libido and erectile dysfunction.
Currently seeking a functional medicine solution.

Thanks for helping out guys, i really appreciate it. Below is all the lab work i've had done since my libido went to zero. I was reading the notes one of the doctors had & he recorded that I mentioned a research article I quoted where nearly 25% of childhood cancer survivors had Low T later in life. I had completely forgot about reading that. I had read that just before visiting my PCP. After visiting with me she said everything I was experiencing was normal & that i'm just getting "old". Which made me so mad I completely forgot about that research article. Is it likely I would have Low T symptoms (and therefore benefit from TRT) with what is considered "normal" levels? Or should I be looking elsewhere for the root problem?

Ok, with SHBG at 31 nmol/L and T at 454 ng/dL one calculator yields a free testosterone of 9.8 ng/dL or 2.15%. The absolute number is certainly low enough to cause problems for some guys.

Don't accept how you feel as normal. You and a competent doctor should decide how far to pursue root causes. Some doctors would order an MRI, though these rarely show much. Oftentimes root causes remain elusive and you just have to address the symptoms. If natural methods fail then there are various options. For as young as you are, low-dose Clomid could be worth trying.

I have the same symptoms and my T has been over 750 multiple times. When he said that if he felt horny then he wouldn't have ED. The first time I didn't feel horny I didn't know if I could get an erection even though I didn't try. But when I did try I definitely couldn't. It was odd being that maybe a week before that I was always horny as usual.

Very true.  But, if you'll notice above, I just suggested he look for root causes.  He's on the lower side, and so I think that's a good idea no matter what.

Have you investigated that cortisol level further? Are you under a lot of stress or anything?

I am not a person that gets stressed easy. It would take a lot to put me under stress, a lot lot. I'm not ruling high cortisol via stress out, but it doesn't line up with my personality. To make it even more unlikely is that it has persisted for a year now (which makes me think if high cortisol is the root cause it's biological, not environmental). I did get tested for Cushings disease & that came back negative (cortisol at 0 after a 1mg Dex dose 8 hours before blood draw). Is there anything else (not stress or Cushings) that could drive my cortisol levels up? To ask that in another way, can cortisol go up without stress or cushings?

I started taking Vit C (1.5 grams) and CoQ10 (300mg) yesterday (3/13). Just to try something. I will try it for 4 weeks.

Just to update this,

I had my Cortisol retested. This time I took 1mg of dexamethasone at midnight and had my cortisol tested at 8 am. My cortisol level was 0. Which is what is supposed to happen when you take Dex, so the endocrinologist said i was normal. Any thoughts? Anyone have an idea on who I should visit? I live in western Kansas, so Denver or Kansas City would be an easy drive.
