Title: Anastrozole Dose recommendation

Just looking for a little advice on this..

On 9-27 My E2 Sensitive hit 9  (8-35 labcorp range) This was with about 1mg anastrozole a week in .25mg doses
On 10-17 Doctor change protocol from 150mg a week to 200mg a week Test Cyp
On 10-19 E2 was 12.4 and between tests I only had 1.75mg total anastrozole
On 11-7 E2 was 15.5 with only .50mg total anastrozole since the last test
Now on 11-28 E2 hit 31.4 and I had no anastrzole between tests. I had a vacation during this last period where I drank a bit of alcohol every day for a week when I normally drink none.

I would say I've been feeling pretty good sexually with E2 between 15 and 31. With it at 9 I was a wreck... do NOT want to go back to that place.
My trough day Total test is 800 with Free T at the top of the range. I hit 800 whether I'm doing 150 or 200mg a week. My body seems to eat it up quick.

I'm thinking of adding back in anastrozole at .25mg the day after my injection and retest in 3-4 weeks. I would rather slowly creep up on the dose than overshoot again (yikes!)  Or should I wait until some type of symptom presents itself?  I'm torn on what to do here. I feel like I've been in a sweet spot for awhile and don't want to mess it up : ).

Thoughts?

If you feel good why would you want to change anything? I think subjective well-being would trump the reference range but I am no expert. If it aint broke.....

Well, it will come to no surprise to the veterans here but despite the advice not to, I tried .25mg of anastrozole once a week, the day after my shot for 3 weeks despite 200mg per week of testosterone, it dropped my E2 down to 11.6... and this test/number was before the 3rd dose so I'm sure it crashed further than 11. I had the same ugly symptoms I did when it had dropped to 9. Mood suffers big time and while erections can still be had, there is no pleasure there.  I started to feel the effects the day after the 3rd dose but from previous experiences I know when my e2 crashes there is a delay of about a week before the symptoms present themselves. I got tested last week on Monday but due to the holiday and time it takes for the labwork I had to sit and wait/worry about where the number was all of last week and through the long weekend. Finally got the result today and am actually relieved because that fix is relatively easy. Just cut out the AI and wait for things to return to normal.  My plan is to not take any AI until I feel like I have symptoms of high E2 for at least a week or so. I may l also get tested every 4 weeks or so to keep an eye on the number and maybe shoot to keep it under 60 but I almost feel like I really need to find out how wide my sweet spot is and just let it get a little high once (if it will even do so). Before starting TRT I had an E2 of 54 on a scale that goes from 0-80 with the TRT clinic. Not sure what test they use but guessing its not sensitive. Since using the Labcorb sensitive I've seen values from 9-31.

Right now I've felt pretty good anywhere from 16-31 so I know that much.


Its really crazy how the low e2 hits me all at once after a delay of about a week. One day, I feel like I'm 18.. libido through the roof and doing great. Next morning blah and libido totally gone. Its also interesting how damn good the libido is when my E2 is on the way down. I keep a daily log of a lot of stuff and how I'm feeling and I've been on fire just before it crashes each time. That may give some weight to the idea that about 20 is perfect in my case or maybe there's something to E2 dropping that causes a temporary spike in libido. 

One thing that perplexes me a bit is that when starting TRT I had that test of E2 of 54 (scale 0-80) and then after 6 weeks on 150mg a week and about .75mg anastrozole a week my E2 only dropped to 48. So according to their test at this time anastrozole wasn't doing a lot for me at that dose.  Now after being on TRT since July I seem to either be really sensitive to anastrozole now or my body just isn't aromatizing near as much any more. In any case I can't help but think I never needed the AI to begin with and who knows how much better I could have felt. I had some good days after starting but they were infrequent/irregular. More recently when being off the AI for 1 month solid I only had 4-5 off days and the others were pretty damn great as far as libido/performane goes.

I've been working with one of the more well known TRT docs mentioned on here since Oct and he's mentioned I may be that rare case that can handle the 200mg a week dose without needing the AI. I'm going to have to find out just how long I can run 200mg a week before the E2 is a problem. I'll be sure to report back.

I think your major issue is your E2 labs not being sensitive assays, if you base you AI dosing on a non-sensitive test you are basing your decision on faulty information. Those non sensitive E2 tests are useless as there are designed for the opposite sex.

I've seen this reaction reported in various forums, and experienced a version of it too. So I think it's a real effect, but I'm not sure of the reason.

Just a little update for those interested. Feeling like my E2 has rebounded some. Just like last time, about 10 days after last AI dose I feel good again. For me its a slow climb and then just like when libido vanishes from one day to the next, its back pretty suddenly too and sensitivity keeps increasing.

Based on my previous lab results when i've been too low I can estimate that my E2 rises about .75 points per day with no AI. Going from around 15 to 31 took about 3 weeks. It may not be linear.. don't have enough data points but at least I have some idea where I'm at.  It looks like for me I need to be at 15 or more to have anything going on down there. Below that, its like its not a part of me and I'm depressed, anxious and irritable. Just terrible.  My trough day Total T is around 800 and Free T is right at the top of the range. SHBG is mid 30s. Prolactin 11. I've never had the joint pain most report with low E2. Maybe I would if I was low for a longer amount of time but I have no plans to find out ; ).

So now we'll see what happens with 4 weeks or more of no AI, hoping it will find a place around 30 and just kind of level out without any AI. I really hate that stuff at this point.

Yeah, you need a certain amount of estradiol for endothelial function to be decent.  And - trust me - low endothelial function will make you feel terrible.  Low estradiol can also lead to bone loss if it goes on long enough as well.
