I have read that TRT can cause or worsen sleep apnea. How and why does this happen? Just curious!

Yes, this is a reported side effect and I noticed that when I went on cypionate I got a little of this.  Of course, it's hard to know if it was from aging or what.

Anyway, I think it is because DHT can alter the throat and vocal cords a bit, but I have never seen any research on that - just my theory.  And I would add that this is not even often a side effect listed by the T manufacturers, so there's not a lot of discussion out there.  In fact, I remember talking to my clinic about it and they said that it should help apnea, not hurt it.  I thought that was an odd statement but just let it go...

Hormones can affect where fat ends up, perhaps T increases the risk of fat ending up around the throat?

I know that some bodybuilders get sleep apnea even though they are young simply from the muscle around the neck.  That must be my problem.   ;D
