UPDATE:

Received blood results back from pathology, results as follows:

Total Testosterone (Centaur): 6.7 nmol/L (8.3-29)

FSH: 2 IU/L
LH: 2.4 IU/L

Prolactin: 162

Dr advises that Gonadtropins are normal so there is nothing to worry about?

I dunno guys, my gut feeling is telling me there is something wrong. Iíve set up a second opinion with a completely different practice just to be sure. Neurology around here requires a referral so Iím sort of stuck there.

Which country do you live?

In my experience, getting married or having a long term girlfriend (no matter how good looking she might be) is a killer to the libido.
Many men need to switch girlfriends every few months in order to keep libido elevated. When you see the same naked female body day in, day out you won't be nearly as interested in having sex with her as were initially.
Personally, I will never get married for this reason alone.

I agree with what the other guys said. If your really wanted to use hCG while travelling, an alternative to Pregnyl is Ovidril, which doesn't need refrigeration

What be the advantage of splitting the daily dose in half and applying twice a day instead of once? Less estradiol, perhaps? Anything else?

Does DHEA help boost libido in guys on clomid mono therapy?

I'm not on an AI. Just 0.5g of a 100mg/ml compounded cream brought my TT level to over 700. Before TRT I was around 300.
Unfortunately, my estradiol is at the bottom of the reference range and I'm not taking an AI.
I don't know what my cortisol level is. It was about mid range before TRT.
The only other thing I'm taking is 2.5mg/day Cialis. Could that cause all the muscle pain?

I'm wondering if would be beneficial to take a break from hCG from time to time or even take it every second or third week? Can it lose its subjective benefits if it is taken every day for months and months or years on end? When I was taking it every day, it lost its subjective benefits fairly quickly and my balls even began to shrink on 100iu ED. When I switched to 500iu twice a week, the balls came back nice and big very quickly. Still, I have reservations about taking 500iu twice a week for the rest of my life. Perhaps it would be better to take it every 2nd or 3rd week?

On the bright side I am hoping stem cell therapies could be an option that work for me down the road.

I know it's a ways away but that is something I am holding some hope for.

You are an eternal optimist, Tyler   ;D

What's severe to you? Can't walk? Can't get out of bed? Soreness is price of the gym...not sure what say here. You're overdoing? Overtraining? If you can't handle the gym then back off from what you're doing? More protein? Stretching? Foam roller?

Basically, I'm taking 5-6 full days to recover from exactly the same workout that I recovered from in 3 days before TRT and vitamin/mineral supplementation. I'm disappointed that TRT has not given me any workout benefits. No increase in muscle strength or endurance at all.
I have read that antioxidants can actually increase, not decrease recovery time, so perhaps that has something to do with it.

Just wondering how your sleep has been? Sleep plays a huge roll in recovery. Iím wonderimg if something in your protocol has changed your sleep quality.

I'm actually sleeping longer at night, but I'm feeling more tired during the day, often needing to take a nap after lunch. Perhaps I should get a sleep study done.

Do you have any pre TRT labs? Maybe fine tune cortisol, thyroid, or e2 to those prior levels will give you the sensitivity.

Maybe you could push shbg up. All the TRT is doing is pushing that down further.

And I think Dr. Crisler is welcoming your case.

Tyler already consulted with Dr Crisler years ago.

The testicles are shrinking because they are not getting the LH and FSH they are used to, as your own system is inhibited by the TRT.

You can replace the LH with its mimic, HCG. I start my guys at 100iu, SQ, per day. Second best is 200iu, every other day. Third best is 300iu every third day. It's a lifestyle decision.

Many guys just do 500iu, twice per week.

Response is variable.

I noticed shrinkage on 100iu per day, but when I switched to 500iu twice per week, my boys came back as large as ever. Is my response unusual?

When I started on the gels in 2012 I noticed I felt hungier, stronger, more energy, and more drive to accomplish my goals.
I went back to school completed an mba got a kickass job and I felt like I had life by the horns.

The gels stopped working for me around 2014, so I tried switching to injections, and same thing they worked for about a year after much tweaking then again same thing, i lost all benefits. Tried pellets. Tried Beta Hcg. Tried patches.

Well I am back on the gels only and it barely feels like anything.

NO hunger.
NO viality or energy.
NO muscle mass.
NO drive.
NO alpha feeling.

The only reason I take it is it is SLIGHTLY better than being off treatment. Not by a lot.

Is this androgen resistance after awhile on T?

What other causes could it be? My endocronoloigst has no idea. I have flown out to see Dr Mariano and he has no idea.

At this point its like well, i have no idea either.

I stopped trying to figure it out until recently and just kept slathering the gel on but its like there MUST be a reason for this.

Some other facts about myself:

My thyroid is fine
I have low SHBG
I am SLIGHTLY pre-diabetic (but still within range)
Gels worked really well for a few years
Injections make me moody
I started TRT at 30 with a total T of 270ng/dl
I tried to come off TRT earlier this year and my natty T was around 200ng/dl so i decided that wasnt liveable.

If anyone has any ideas i would appreciate it. It's driving me crazy that it used to work and now it doesnt. If it NEVER worked then it would be like ok it never worked but i KNOW what good feels like and now I cant seem to get back to that feeling.

Oldbull aka Tyler81? :)

I've been on TRT for 3 months now and even though my T level is near top of the reference range, I have not experienced faster recovery from workouts or an increase in muscle strength or endurance. What's been bothering me the most is severe DOMS. It is worse than before TRT. Apart from TRT, the only things I have changed is taking a multi vitamin/mineral supplement (Enzymedica Enzyme Nutrition Men's multi) daily and drinking home made milk kefir daily. Could any of these things be contributing to my severe DOMS? Is there anything that can be done to reduce DOMS?
