Peak i am 44 ...sorry i confused you lol , but this shrinking, wrinkly ,lifeless, hourglass  shape (sometimes)  of my penis suggests poor penile  bloodflow and  androgen deprivation ...I am worried that it may deteriorate my penile tissues further.
As i said i see much older men (no disrespect to anyone) probable in their 60's  at the gym but in a flaccid state their penis looks fuller and of more "substance" than mine .
For that  reason i think i would like to boost my T level via HCG this time not Clomid and  just to maintain or improve my penile tissue...whats your take on that...i will only do this once ive completed my Naltrexone protocol...meanwhile havent seen any improvement yet
49 yrs  suffered ED/weaker erections from  24 yrs ,  used Viagra/Levitra  successfully since 1999/2003 until they suddenly stopped working 7 years ago ,after i met and fell in love with woman of my dreams . My T levels are great but ED is still  severe  ..there seem to be no known cause tried several specialists.Symptons Penile tissue shrinkage extremely poor arousal ..i guess effects of aging

Davie12,  between 20-30 i didnt know of any hypertension  i wasnt  that aware of hypertension and probabably didnt think i suffered any symptons ...any regular checks with my BP wasborderline or not too bad , but after 27 or so i became overweight and could possibly have had mild BP...but  i havent  been treated for BP(hypertention).
Over the lst 4 years i have been physically active and my blood pressure has been OK , but i will keep checking....so what are your suggestions please..

The reason I was asking is that you mentioned something about that previously. The problem with hypertension is that it is a silent and normally asymptomatic problem...similar to high cholesterol. In other words, you can live with it a long time and not even know you have it, unless it is detected through a physical exam. I've heard that untreated high blood pressure for many years can lead to a whole host of problems. Keep in mind blood pressure has to do with the elasticity of arteries/veins and ease of bloodflow, and the genital area is just like any other vascular part of the body in this regard. I assume that arousal would still be there if everything is OK hormonally, but the penis wouldn't respond properly (unlike hormonal problems where the arousal is also compromised). Incidentally, the best way to greatly improve blood pressure and elasticity other than diet, of course, is high intensity aerobic work (ie. sprinting), as it pushes the blood vessels to dilate significantly, thus make them more elastic over time.

Well, if I remember right, you are medium on testosterone.  I'm not sure how much raising your T further will help but you can try.  The problem is that you can get extra estradiol as you increase which negates any benefit.

Your issues are definitely the most enigmatic on this board...

Wounded Lion...Your problem is definitely not T related as there is nothing wrong with your levels and they are in fact probably at the high end of normal for your age.

You have a blood flow and possible nerve problem that is for sure as I had exactly the same problem after the small op I had which resulted in my penis becoming lifeless and cold. You should really be properly looked at to find out if there is any nerve/ tissue damage before taking any other action.

My personal recommendations are to carry on trying boost your blood flow down there as much as you can by increasing NO with daily citrulline, POM and the rest. As we have mentioned on here  before it is essential to get blood flow /nutrients to where it matters most if you have had problems with no erections it will be essential to try to keep on feeding your tissues and assisting any nerve regeneration by mega dosing with the correct nutrients .
I don't think the penile injections will be doing you any favours either.
 
I am slightly puzzled by your scenario because you have not had any trauma (unlike myself) that could have contributed to your situation in the first place. What I can say is that you can heal...as I have
